i haven't played a game with food allergies that also required food for survival. it would be an interesting twist.

Yeah, no. No RNG weakness to a necessary resource. There would be an "optimal" allergy that everyone would just re-roll for, and it really wouldn't add anything even for a hardcore character. Just RP it if you want your character to die from peanuts.

There will be a best food in the game for a given character build and you'll want to reroll to be allergic to some other food that you'll consequently never eat.

Yeah some things - while they sound cool in theory - don't add to the gameplay experience and end up just being a nuisance for most folks. Those things are best left out. Just my 2 cents.

Might as well ass phobias into the game too then. Getting paralyzed when seeing spiders. Afraid of water and you can't walk through water, no matter how deep. Get random fear debuff where you run around with no ability to control yourself during night, because the fear of the dark.

What about claustrophobia, you can build your house, but you can never enter it.

Why? The slim majority of people don't have food allergies. Where noting that some do is important in real life, making everyone have one in game feels more like some gimmick, since it isn't really so widespread.
Now, I could see different worlds having different foods, and potentially having that play havoc with groups that lose supplies before they are ready to go back. It would make logistics for such a trip even more important. But just a general food allergy for everyone is at the very least over-kill.
Having negative to characters can be interesting, but I'd want to see a lot more variety than just food allergies.

what if food doesn't give buffs outside of replenishing the "you need to eat" bar.

how many do have them but are undiagnosed? I'm lactose intolerant and severely allergic to bananas. the banana and lactose were never diagnosed by a doctor but something I found/figured out along the way.

air is a big thing just gota find out which component of air it is that couses the alergic reaction and filter that out =P
