Permanent flaws

I would like the option at character creation to choose a permanent flaw that would affect gameplay to a small amount. An example would be having one eye and wearing an eyepatch would give you less awareness but boost something else. I really like things like this in rpgs for flavor factor. What ones would you guys like to see?

If you have a splint at your leg, you can just walk slower, but you will be able to see more and to notice more about your surrounding area.
If you have the cold, you are groggy... but you will infect all people around you as long they don't have a saving mouthpiece.
If you have a glass, you can use it to make fire.

The problem with this system is that it allows people to take flaws that are largely irrelevant to their character and presumably be compensated accordingly, basically getting extra power for nothing.

@fibs i think it should be more like a gimmick.
This small things that makes you smile and laugh when you use/notice them.

@fibs If the benefits are small enough then it really wouldnt matter but would still satisfy the idea. I mean hitting for 1% more damage or getting 1% more gold at the end of the day is near meaningless.

@dagimir of if you wear your beloved talisman on your neck, you get a better mood and feel you can carry tons of trees!
(shown as you would carry much bigger logs than they are.)

I thought it would just slightly affect raw stats, like pokemon natures or that fire emblem boon/bane system. Doesn't have to be a major change in stats.

This would be difficult to implement in a balanced way, but the premise is interesting!

I think it's an interesting idea. You could have all sorts of negatives like being melancholic which could affect how you talk to NPC's, but also offer a slight buff against mind altering spells.
Just need to keep the buffs/debuffs really limited and weak so it's not OP.

I really likes the thought. It's kind of like the backgrounds in the old RPG Arcanum from Sierra. Nothing game-breaking, but added a lot of flavor.

Lets say a flaw of having no hair, so you are less resistant to cold environment, but there is a chance to blind your enemies in combat.
