I think it will be only given when the game releases.
The title is Forum title, you can already do that in your forum profile.

The titles from the Packs don't exist yet.
You can set the title of your Foundation level in your Profile:

Yes, the only titles that are in the forum at the moment are the Foundation titles. You get the first one with Foundation level 5. You can find The Foundation here.

Check the "Quests" Tab and it will tell you
You can get to like level 60 or something by clicking on all the news posts and pressing the share on Twitter and Facebook buttons.
.Other ways to get points are doing your Daily Post Quest, reading newsletters and participating in the weekly drawings and stuff.
