Ultima Online had PvPickpocketing and it was hard as balls to do. You had to very obviously sidle up to your mark and they would be alerted most of the times you tried. If I recall, you were not able to steal equipped items.
People pickpocketed less over time because you also could not steal blessed or insured items and it became way way too easy to get those as the game went on.

. There are no NPC thieves guilds, but nothing is stopping you from running your own thieves guild.
